You must be fingerprinted, undergo a SLED background check, receive a statement from Social Services indicating whether you are on the department’s Central Registry of Child Abuse and Neglect, submit an affidavit stating whether you are required to pay child support or alimony, and file a name change petition with the court.  The court may require a hearing in which you will need to attend.
